Physician Service Fee

The DIAB Medical Group Service Fee of $10.30 is collected on behalf of DIAB Medical Group and its affiliated professional entities for independent physician oversight of lab testing including ordering testing if appropriate, as well as providing physician follow-up services. Cancellation Policy

You have 60 days to cancel your order prior to completing your testing. There is a non-refundable cancellation fee of $20, which includes the fees charged by DIAB Medical Group and DIAB Management Group Diagnostics administrative fees. In some states, applicable taxes are non-refundable as well..

Expiration Policy

Your order will expire after 60 days if you do not complete your testing. You will be automatically refunded your total purchase amount minus the cancellation fee of $20, which includes the fees charged by DIAB Medical Group and DIAB Management Group Diagnostics administrative fees.
